Upgrade Labs Waiver

I, acknowledge that there is a risk associated with participating in biohacking services at Upgrade Labs (“Upgrade Lab” or “Facility”). My participation is completely voluntary. By using the Facility I acknowledge that I am assuming all risks of injury or illness, due to conditions of the Facility, others, or my medical condition. I agree on my own behalf and on behalf of my personal representatives, heirs, estate trustees or assigns to release, indemnify and discharge Upgrade Labs its owners, officers, directors, agents, employees, independent contractors, and affiliated companies from any and all claims or causes of action (known or unknown) brought against Upgrade Labs by any party arising out of my actions, including my negligence, while at the Facility. Upgrade Labs is not responsible for any damage to, loss or theft of my personal property. I have reasonably concluded that I am physically fit and have no medical condition which would prevent my full use of Upgrade Labs, the Upgrade Labs Services, or the Equipment, and at no time have I been advised by a medical practitioner that I am not fit or would be endangered by utilizing the Upgrade Labs Services or Equipment, or services or equipment that are substantially similar to Upgrade Labs Services.
